REGISTRATION FEE 

 
Registration fee for active members and doctors in training includes:

• Participation in the scientific programme
• Final programme & abstract book
• Coffee breaks & lunches          
• Welcome reception at the Copenhagen City Hall on Wednesday 13 May
• Dinner and dance banquet on Friday 15 May at reduced price
  (DKK 650 for participants, DKK 950 for accompanying persons)

CONGRESS DINNER
If you wish to participate in the dinner and dance banquet on Friday 15 May 2009 the price is:

• DKK 650 for participants
• DKK 950 for accompanying persons.

Congress dinner includes:

Welcome drink
Banquet inclusive wine
Coffee, petit fours and one avec
Free bar (beer, wine, soft drinks)
Midnight snack

PAYMENT
Payment should be remitted as follows:

1. Credit card

American Express, Diners Club, Euro Card, Master Card, and VISA are accepted. If you are registering on-line, the only credit cards accepted are Eurocard Master Card and VISA. If you want to use American Express, Diners Club or Dankort (Danish Participants) please complete the registration form and fax it back to fax number: + 45 70 23 78 88. Please remember to indicate credit card number and expiry date, as well as the three (3) digit control number (CVC) which is printed on the back of your credit card. Some credit cards have a four (4) digit code on the front.

Please remember to add bank charges, as the recipient will not pay those.

Please make sure that your name, address, and “GP2009” are clearly stated on all payments and transfer documents.

CONFIRMATION OF REGISTRATION
Confirmation letter will be e-mailed to you as soon as we have received your payment.

CANCELLATION & REFUND POLICY
Notification of cancellation must be made in writing and mailed to ICS - International Conference Services A/S and with a reference to a bank account incl. the SWIFT Code and IBAN (International Bank Account Number), where a possible refund may be remitted to. An administration fee will be charged depending on the date the cancellation is received by ICS.

Credit cannot be given for unattended events, late arrivals or early departure. All refunds will be processed after the Congress.

If you have sent in an abstract and if it is not accepted for presentation you may cancel your registration free of charge.

Insurance
You are requested to arrange your own health and travel insurance. The congress organiser is not able to take responsibility for any injuries or damages that may occur during the Congress.
